Differential Diagnosis

  1. Toxic shock syndromes (Staph or Strep)
  2. Rocky Mountain Spotted Fever
  3. Toxic Epidermal Necrolysis
  4. Mercury hypersensitivity reaction (acrodynia)
  5. Gen complicated measles
  6. Incomplete (atypical) Kawasaki

Acute Treatment

  1. ABCs, IV, O2, monitor
  2. Medications
  3. Pts may appear to be improving, then suddenly crash
  4. Further Care
  5. Salvage therapy
  6. Resistance to IVIG
  7. Acute coronary thrombosis fibrinolytic therapy - Peds cardio

Disposition

  1. Admit

Follow Up Care

  1. Screening for coronary artery aneurysms
  2. Follow up w/PCP, Cardiologist w/in 1wk
  3. Pts w/o ECHO abnl do not need activity restrictions, medications after 3mos
