Treatment

  1. Treat volume depletion and electrolyte imbalance from any protracted vomiting or diarrhea
  2. Begin antibiotics empirically to treat likely pathogens; these can be po or IV
  3. General surgical consult

Disposition

  1. Admit for observation if still question appendicitis vs mesenteric adenitis
  2. Hospitalize for hemodynamic instability, ongoing vol depletion, signs of sepsis
  3. Admit to surgery for signs of peritonitis, abscess, or if acute appendicitis cannot be excluded
