Treatment

  1. Treat underlying medical illness
  2. NPO, IV fluids, NG Suction, rectal tube, correct electrolytes
  3. Stop anti-motility agents (incl CCB's)
  4. Enemas
  5. Neostigmine is effective in 85-90% of cases
    • Lasts 1-2 hours, multiple doses may be needed
    • Cardiac monitor, atropine at bedside
  6. Methylnaltrexone (Relistor): if opiate-induced
  7. Colonoscopy if medical therapy fails
