Von Willebrand Disease (vWD)


Pathophysiology

Diagnosis

Treatment

  1. DDAVP (desmopressin) alone in mild-mod type I VWD (most common) 0.3mcg/kg IV over 15-30min
  2. DDAVP nasal spray, one in each nostril for minor bleeding
  3. Factor VIII concentrates containing large amounts of VWF (Humate-P and Koate-HS) for types II and III VWD
  4. Correct bleeding time w/cryoprecipitate or FFP to <10min if minor bleed or <5min if major bleed for all types VWD; carries increased risk for dz transmission
  5. Platelet transfusions if refractory bleeding

Disposition

  1. Admit for major bleed/new dx
  2. Good prognosis
