Toe Fracture

  Initial Treatment
Splint type & position Buddy taping to adjacent toe
Consider SLWC with toe platform for great toe fracture
Initial follow-up visit Within 1 to 2 weeks
Within 3 to 5 days for open fracture to assess wound
Pt instructions Elevation and icing essential to reduce swelling
Hard-soled or postoperative shoe
  Follow-up Care
Cast or Splint type & position Buddy taping with hard-soled shoe
Length of immobilization Buddy taping: 3 to 6 weeks
SLWC with toe platform: 2 to 3 weeks followed by buddy taping and hard-soled shoe
Healing time 4 to 6 weeks
Follow-up visit interval Every 2 to 4 weeks
Repeat radiography interval 7 to 10 days for intraarticular fractures or fractures requiring reduction only
Pt instruction Persistent pain possible for several weeks or months
Hard-soled shoe worn as much as possible during healing
Indications for Ortho consult Displaced intraarticular fractures
Fracture dislocations
Intraarticular fractures involving >25% of the joint
Unstable displaced fracture of the first toe
Open fractures of the proximal phalanges
Open fracture of the distal phalanx with gross contamination or delayed treatment
