Septal Hematoma


Pathophysiology

  1. Collection of blood between mucoperichondrium & cartilage of nasal septum
  2. May cause airway compromise
  3. If left untreated, can result in septal abscess, necrosis, &/or deformity

Diagnosis

  1. Grape-like swelling of nasal septum
  2. Feels boggy when palpated
  3. Suspect if patient is s/p facial trauma
  4. Nasal X-ray (Waters view) may be helpful

Treatment

  1. Primary trauma survey
  2. Procedure
  3. Antibiotic treatment (toxic shock synd. prophylaxis)

Disposition

  1. Admit if any significant trauma issues
  2. Follow up with ENT / plastics 1-2 days
